  • 简介:   [摘要 ] 目的 评价应用小剂量胰岛素持续滴注治疗小儿糖尿病酮症酸中毒临床效果。方法 收取该院 108例糖尿病酮症酸中毒患儿,收取时间在 2017年 10月— 2018年 10月,并将糖尿病酮症酸中毒患儿分为 2组,对照组( 54例患儿实施常规胰岛素剂量治疗),观察组( 54例患儿实施小剂量胰岛素持续滴注治疗),将 2组糖尿病酮症酸中毒患儿的治疗效果进行对比。结果 观察组糖尿病酮症酸中毒患儿治疗后的酮体转阴时间( 18.15±8.20) h、血糖下降速度( 5.15±2.7) mmol/h、餐后 2 h血糖( 8.01±1.05) mmol/L、空腹血糖( 7.01±1.12) mmol/L优于对照组( P<0.05),观察组糖尿病酮症酸中毒患儿治疗后的症状完全消失时间( 5.29±1.18) d、住院时间( 15.23±1.27) d优于对照组患儿( P<0.05),观察组糖尿病酮症酸中毒患儿中,显效患儿有 50例、有效患儿有 2例、无效患儿有 2例、总有效率 96.30%高于对照组患儿( P<0.05)。结论 对糖尿病酮症酸中毒患儿实施小剂量胰岛素持续滴注治疗,具有十分显著的治疗效果,能促进患儿较快康复,值得临床进一步推广和应用。     [关键词 ] 小剂量胰岛素持续滴注;小儿糖尿病酮症酸中毒;临床效果    [Abstract] Objective To evaluate the clinical effect of continuous infusion of low dose insulin in the treatment of diabetic ketoacidosis in children. Methods 108 cases of diabetic ketoacidosis in our hospital were collected from October 2017 to October 2018. The children with diabetic ketoacidosis were divided into two groups. The control group (54 cases were treated with routine insulin dosage) and the observation group (54 cases were treated with continuous infusion of small dose insulin). Two groups of diabetic ketoacidosis were treated with continuous infusion of insulin. The therapeutic effects of children with ketoacidosis were compared. Results In the observation group, the time of ketone body turning negative (18.15 +8.20) h, the rate of blood sugar decreasing (5.15 +2.7) mmol/h, the blood sugar (8.01 +1.05) mmol/L and the fasting blood sugar (7.01 +1.12) mmol/L after treatment were better than those in the control group (P < 0.05). The symptoms in the observation group after treatment were better than those in the control group (P < 0.05). The time of complete disappearance and hospitalization of diabetic ketoacidosis in the observation group were 5.29 (+1.18) D and 15.23 (+1.27) d better than those in the control group (P < 0.05). Among the diabetic ketoacidosis patients in the observation group, 50 were effective, 2 were effective, 2 were ineffective, and the total effective rate was 96.30% higher than that in the control group (P < 0.05). Conclusion Continuous infusion of low dose insulin in children with diabetic ketoacidosis has a very significant therapeutic effect and can promote the recovery of children, which is worthy of further promotion and application in clinic.
